Responsibilities:
* Conducting assessments
* Creating bespoke outcome focused care plans
* Building and maintaining professional relationships with clients and other stakeholders
* Maintaining accurate and up-to-date files
* On-call duties
* Providing care in community
Requirements:
- At least 1 year of Care Coordinator experience in a domiciliary care environment,
- NVQ Level 3 in Health & Social Care,
- Full UK Driving licence (Must have own car)
You will be required to complete all training as requested by your line manager. You will be expected to work within the NICE and Local Authority guidelines.
Working Times:
Monday – Friday: availability 7am -10pm but not required to work 15 hour shifts.
Every other weekend, you will be required to be available between 7am and 10pm.
